Title : Plasma microRNAs levels associate with the outcome of sepsis-induced ARDS patients


Background and objective: Mesenchymal stem cells (MSC) can alleviate the damage of structure and function of pulmonary vascular endothelial cells (VEC). But therapeutic effects of MSC are different among sepsis-induced acute respiratory distress syndrome (ARDS) patients. MicroRNAs (miRNAs) play important roles in MSC regulating function and activity of vascular endothelial cells (VEC) which may affect therapeutic response and outcome, but few studies focused on this field. The purpose of our study is to find specific plasma miRNAs regulating VEC function and associated with MSC (MSC?VEC?miRNAs) associated with the outcome of sepsis-induced ARDS patients.

Methods: We obtained MSC-VEC-miRNAs through searching PUBMED database. 56 sepsis-induced ARDS patients were enrolled within 24 hours of admission to ICU. Clinical data were collected. We tested plasma levels of MSC-VEC-miRNAs by real-time fluorescence quantitative PCR. Plasma inflammatory factors TNF-α, IL-10 and endothelial injury markers VCAM-1, vWF were detected by ELISA method.

Results: Fourteen MSC-VEC-miRNAs were identified after screening. 56 sepsis-induced ARDS patients were included in this study, among which 18(32.1%) died on Day 28. Plasma miR-155 level and miR-320 level in non-survival group exhibited significantly higher than those in survival group (0.42[0.08-1.99] vs. 0.12[0.03-0.32], P=0.036; 0.97[0.23-2.57] vs. 0.32[0.11-1.10], P=0.032). Plasma levels of IL-10 and TNF-α in survival group were significantly lower than those in non-survival group (1.39[0.88-2.00] vs. 2.10[1.14-5.65], P=0.020; 2.72[2.19-3.69] vs. 2.92[2.37-4.30], P=0.047). Other 12 kinds of miRNA and plasma vWF, VCAM-1 levels in plasma between two groups showed no statistical difference.

Conclusion: Plasma miR-155 and miR-320 levels have a certain predictive value for the prognosis of ARDS patients.
